Agreement: this paperwork is solely for when ALL parties can agree on a custody schedule and no court intervention is needed. This paperwork will need to be notarized with ALL parties' signatures.
The completed document is to be filed in Prothonotary; if there is an open case, there is no fee, but if there is no existing case, there is a $126.75 fee.
Petition to Intervene: this paperwork is solely for when someone wants to be included in an existing custody action or case.

Continuance Request: this paperwork is to request to reschedule a hearing or conference date; ALL parties to the case must be notified of this action.

Withdraw: this paperwork is solely for the purpose of cancelling an action or closing a case, meaning Court intervention is no longer needed.
The completed document is to be filed in Prothonotary; if you are canceling an action, there is no fee, but if you are requesting to close a case, there is an $11 fee.
